Rearrange Items does not work sometimes???

I have had a few instances lately, on 2 separate installations, where Rearrange Items does not work.

I understand about doing a complete browser shift-refresh after a change. The weird thing is that the displayed arrangement in Arrange Items is correct, but the generated page is not. I have tried deleting the template and database caches.

Anyone else seen this? Any fixes / workarounds.
