When my host changed from MySQL4 to 5 I don't recall doing anything, everything literally just worked, same with the transition from PHP4 to 5, and FreeBSD 4 to 6. But my host isn't your typical host, they don't just use a GUI to point-n-click upgrades and installs or type in apt-get install or apt-get upgrade and hope everything works (seems that's what most of these plesk or cpanel bases hosts do). They actually know how to use and configure the operating system, servers and other software running on their servers that they hand build in their data center that they own.

So with your typical host, who knows, make sure you have backups :)
