Module contribution and author association

I have published 2 modules and 1 theme on the Codex. But when I try to see all of them from my control panel http://codex.gallery2.org/User:Serge_D, only one is actually there

How do I make them all visible and available for management from one place?

You need to edit that page.

You can write a module to do that for Drupal. :-)

It's mediawiki actually and according to ckdake, customizing mediawiki is a real headache. When I was creating that template that's used on those pages I searched everywhere to see if you could grab the property of the pages to see who the first person was to create it...
