If I choose image action and then rotate, the thumbnail is rotating but the actual picture isn't.

Gallery does not actually change the image's embedded data, preserving the original image.
Gallery is only recording the rotation for it's own display.
Lightbox shows the raw image without gallery intervention, thus would not show the rotated image.
If you used a theme without lightbox it would be rotated.
If you wanted the image to be permanently rotated, do it before upload, or rotate then re-upload.
