Add Comment Form to View Photo Page

I peeled through the forum for some time looking for a way to achieve this. I want to have the add comment form displayed right in the page. If sounds like the someone almost got it in the thread below, but I can't quite find where everything is supposed to go. Always, I don't know if things have changed since the versions have change since this was last posted.

I also have a question about the Comment Box. Can I change the "leave Name or email -- To leave name and URL. So that the name then becomes a hyper-link to their site. This way I can view their site too.
I did search this forum - but if this question has already been posted - sorry.
