Trouble with extra fields .. I think

I recently changed hosts for this domain. I've had the extra fields patch installed prior to moving the domain. After I finished transfering files, I ran the setup Wizzard and came across the messages listed below when I got to step 4

Error: \"Location Type\", \"Location Style\" is not of the form "field one", "field two", "field three"

I went ahead and compared the functions.inc file with my other domain and I have the extra fields patch installed there too and the file from what I can tell is identical.

Any help would be greatly appriciated
