@3, they don't exist. The theme structure has been completely changed from B3. The theme/layout structure in B3 is obsolete.
B4 is going to be released very soon. There are 4 or 5 themes that ship with it. Currently I only know of one other user theme that's been updated for B4 and that's Siriux. Search this forum for the posts by the other about that theme.
@2, see my first answer. When you upgrade to B4 the file you edit is different that what you'd edit in B3. I don't remember what that file was in B3 and I'd have to dig around in B4 to find the file.
@1, you can hide the "updated" "button" completely, but it would be for everyone, even the administrator. Site Admin > Modules > deactive New Items. If you still want that button when something is new, then try Site Admin > New Items > and entering 0 for "Updated".
