File Upload customization

I Cannot believe I just spent several hours looking for this with no luck.
Can anyone help me please?
I need the upload form to have custom fields.
A new member comes to site.
Logs in, wants to upload a photo.
Puts pic in,
custom fields:
Name, City, State, Country, website etc.
Clicks upload, file is uploaded and information is displayed under it.

This seems like a normal thing to have. I am totally at a loss as to how to get it or why it isn't available. Can pay, whatever for this, need help desperately.

I have both, heard it was easier with v1. I don't care which version can do it, we are desperate

Try out Gallery 2 with the custom fields module. I think it adds it to the upload page.

Is this some strage request?
To me, the ability to upload a photo with additional information is something like having sliced bread, not unusual at all.

Someone has a photo gallery. They want to upload a photo to it, and with the photo the uploaders name, state, website etc.

Can this be done? Am I being weird? Is this an "open sourse" problem?lol

It's not a strange request. Other people have requested the same thing before. It can be done but a developer has to have time to do it and interest in it for it to get done. If you would like to submit a patch or find someone to write and submit a patch, we will gladly take it but the most you can do aside from that is (after searching for an existing one) post a new feature request on http://sf.net/projects/gallery/ encourage others to vote on it at http://gallery.menalto.com/sfvote and wait for a developer to get around to it.

I find it difficult to understand the fact that it hasn't been done. I first used gallery some years back...wow

Gallery 2 is new, everyone has been working on it, other parts of it have been more important than adding custom field stuff on upload
