I need a customized galery search based on some previous customizations, needs to stand alone, more work coming if this works out.

I need a search that looks like this:

at the top of each page, and that includes the following functionality.

- The categories are pulled from a data base including all the categories used on the site.
- The zip code radius searches for all zip codes in that radius
- The clicking search returns results much like the site all ready does.
- The ultimate formatting should, have what is now "search the gallery" replaced with this and should be accross the top (not on the side)
- One version of search needs to stand alone and I need to add html and graphics to the page.

All of these items are already functional in different places on the site, I merely need them in the search engine. If any of these are show stoppers or especially tough, let me know and I'm willing to comprimise.

Need fast turn around & fixed rate, I am flexible on solution approach.
