Database Setup (stuck need help)

I Installed the version 2.1and i got stuck setting up
I need help with the following:

DB Hostname: " is that the info from my server?"
DB Username: " is that the info from my server?"
DB Password: " is that the info from my server?"
DB Name: ?
Table Prefix: "WHERE CAN LOCATE THAT?"
Column Prefix: "WHERE CAN LOCATE THAT?"

first try README.html for help.
leave the defaults for table/column prefix.

I have been trying to figure out this step for hours and no luck....
I read the README.html for help. And i was not able to find the solution for the problem
here is the error that i'm getting:
Warning: mysql_connect(): Unknown MySQL Server Host '//host482.ipowerweb.com' (1) in /home/directpa/public_html/Gallery/lib/adodb/drivers/adodb-mysql.inc.php on line 348

Can you point me in the right direction?

I reinstalled the old gallery
but the original problem it is still there

here the error:
Error: Unable to make thumbnail (0)
