Add Captions/Keywords field when uploading picture

Have anyone customized [ Keywords field ] while posting picture successfully?? please plzz share .. :( I want this real bad.. Keywords available at the time members uploading photo, instead they have to go back Edit photo to updates keywords.
I'v tried in many ways and followed many threads around this board but no luck at all. It didnt inserts to database.

* [Caption field]
* [Keywords field] ?

any help would be appreciated..
